I gave this a bad review at first but once I started playing some of the DLCs and found that some of them match very well.

Purely cosmetic and a little expensive for how little is included. Really just look at the store page, if you absolutely must have what you see to play dress up with your doctors, go ahead and buy it. I just can't recommend this one at this price.

It's $5 for cosmetics, but very obviously $5 for cosmetics. I like using some costumes to to easily keep up with certain specific staff members. If I am trying to train a level 5 specialist, you better believe it has a custom name and new outfit so I do not accidentally shove it in a different class. Other than that, I do just like the cake stand and what not. Certainly not necessary, but I would not consider it a waste of $5 either.
